By submitting this application, I certify that the above information is true and correct to the best of my knowledge. I also agree to:
---Attend 90% of all sessions for the Project Blueprint Board Leadership Training Program, including the orientation, training sessions, and nonprofit board speed dating. I understand that missing more than one session means that I become ineligible to receive the stipend.
---Use the knowledge gained through this program to actively volunteer with a nonprofit located in or serving constituents in Forsyth County.
---Submit a W-9 if accepted into this program.
---Develop an action plan for stipend expenses in accordance with the guidelines outlined in the document "Professional Development Activities." I understand that the action plan must be approved by HandsOn NWNC prior to receiving this stipend.
---Complete the training evaluation at the end of each class and a final evaluation of the overall effectiveness and impact of the program once the stipend has been spent.
